Application for Adjustable Rate Mortgage Loan
If you are considering an adjustable rate mortgage loan, you can:
  • Accept the Disclosures and Apply online
  • Accept the Disclosures and Download application

  • Visit any branch to complete an application.
Disclosure Information
The Consumer Handbook on Adjustable Rate Mortgages (PDF) is a publication provided by the Federal Reserve. It contains important general information about adjustable rate mortgage loans.

Download applicable disclosures electronically. The following disclosures contain important information about adjustable rate mortgages and the specific programs offered by NSB. Please read them carefully. We also suggest that you print the documents and maintain them for your records.


However you apply for your NSB mortgage, you'll need to have some information at hand. Here's what we'll need:
  • Your last two pay stubs
  • Your W-2 income statements for the last two years
  • If self-employed, provide personal and business tax returns for the past two years, and your year-to-date profit & loss statement
  • Recent statements (covering a 2-month period) for your investment and bank accounts
  • Institution names, account numbers and addresses for all assets (bank accounts and investment accounts) and
  • All liabilities (loans of all types, including credit cards)
  • A copy of your Purchase and Sales Agreement, if you are buying a home. In addition, you will need to complete the Mortgage Application. This is available by choosing one of the options above.

Important Information About Procedures for Opening a New Account
To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.
